Description
T3 Uptake is a thyroid lab marker that helps estimate how much of your thyroid hormone is bound to carrier proteins (especially thyroid-binding globulin, TBG) versus how much binding capacity is “available.” It is not a direct measurement of free T3 in the blood. Instead, it’s most commonly used as part of older-style thyroid interpretation (often paired with Total T4 to calculate the Free T4 Index).
Why it matters
Most thyroid hormone in blood is bound to proteins. If binding proteins change (pregnancy, estrogen therapy, liver disease, illness, certain medications), Total T4 can look high or low even when true thyroid function is normal. T3 Uptake helps provide context about binding-protein effects so clinicians can interpret Total T4 more accurately.
What higher or lower results can suggest (general education)
Higher T3 Uptake often reflects lower binding proteins (less TBG), which can be seen with hyperthyroidism patterns, protein loss, androgen exposure, or some medications.
Lower T3 Uptake often reflects higher binding proteins (more TBG), which can occur in pregnancy, estrogen therapy, and some liver or inflammatory states.
How to interpret it
T3 Uptake is best interpreted alongside TSH and either Free T4 (preferred modern test) or Total T4 + Free T4 Index (older approach). If your thyroid testing is confusing or symptoms are present, repeating labs under typical conditions and reviewing medications/supplements with a clinician can clarify the pattern.
